Tiffany 1897 and Tiffany 1910 list "ornamental windows" for the "Broadway Congregational Church" in Norwich, which is now called the United Congregational Church (see web link below). In fact Tiffany 1897 (in the extracts at the back, p. 80) quotes an article from the Norwich Bulletin (no date specified) about the windows and also a carpet and other decorations. Tiffany 1893 (the appendix to the Chicago Exposition booklet) lists the church and so 1891 is a plausible date for the windows, although I have yet to find a (definite) 1891 source which states this. (Possibly the Norwich Bulletin article was indeed 1891.)
The bit about Tiffany making the windows (it seems to imply in the church itself!) is obviously an embellishment.
Back in 2014 the church was put up for sale but it looks like they must have changed their minds about that now.
