St. Hubert's Chapel, Kinnelon, NJ, was a private chapels built by Francis S. Kinney back in the late 1880s / early 1890s. It fell into disrepair and in particular was vandalised in the 1950s.
It has some Tiffany windows and mosaics, and also a large exterior bronze tablet / bas-relief of St Hubert designed by Holzer. It also has seven stained glass windows illustrating the life of St. Hubert made by William Gibson's Sons.
Tom and Corrine Kline, and many others, have been working on restoring it for quite some time now, with the involvement of Rohlf's Stained Glass Studio and others, and they have just launched a new website, with photos of the windows, etc.:
